is devoted to saving people from heart disease and stroke -- America's No. 1 and
No. 4 killers. We team with millions of volunteers to fund innovative research,
fight for stronger public health policies, and provide lifesaving tools and
information to prevent and treat these diseases. The Dallas-based association is
the nation's oldest and largest voluntary organization dedicated to fighting
heart disease and stroke.

American Kidney Fund The
American Kidney Fund
fights kidney disease through direct financial support to patients in need,
health education and prevention efforts. It leads the nation in charitable
assistance to dialysis patients. Last year, nearly 84,000 people --1 out of
every 5 U.S. dialysis patients -- received assistance from the American Kidney
Fund for health insurance premiums and other treatment-related expenses. [+]

American Society of
Hypertension is the largest organization of hypertension researchers and
health care providers in the United States committed to preventing
and treating hypertension and its consequences. ASH has a domestic and
international membership of basic science & clinical investigators, physicians,
physician assistants, nurse practitioners, pharmacists, as well as individuals

Preventive Cardiovascular Nurses Association
The
Preventive Cardiovascular Nurses Association (PCNA) is
the leading nursing organization dedicated to preventing
cardiovascular disease through assessing risk, facilitating
lifestyle changes, and guiding individuals to achieve
treatment goals. The mission of PCNA is to promote nurses as
leaders in the prevention and management of cardiovascular
disease. PCNA does this by educating and supporting nurses
through the development of professional and patient
education, leadership, and advocacy.

Right Care Initiative The
Right Care Initiative is a California-wide, public-private healthcare
quality improvement collaborative formed in 2007 by the Department of Managed
Health Care and the Deans of the UC Berkeley and UCLA Schools of Public Health.
The goal is to measurably improve health outcomes by spreading patient-centered,
evidence-based practices, with a particular focus on preventing and managing
heart attacks, strokes, and complications from diabetes. [+]
The Right Care Initiative seeks to support health plan and medical
group efforts to achieve key national 90th percentile HEDIS healthcare quality
targets: blood pressure control for patients with hypertension, LDL cholesterol
control for patients with heart disease or diabetes, and blood sugar control for
patients with diabetes. Deploying evidence-based practices more broadly will
establish better care management for patients with these chronic conditions,
avoiding unnecessary costs, disability and death.
